When renewing, go to www.palms.org and log in with your member number and password. (If you don't know your member number, email info@palms.org.) Select "renew membership" and a form will pop up with your information. Make any necessary corrections. Toward the bottom, you will see "How will you pay?" with a drop-down menu. To mail in your payment, choose either "Credit card by mail" or "Check" and continue.  The next screen will have the address:

Print this registration form and mail to address below or fax to (512)-870.9366
